If you served this country and are now experiencing homelessness or housing instability — you deserve better. Grace Trace Ministries provides immediate placement, VA coordination, benefits navigation, and a clear path to permanent housing.
♿Individuals with Disabilities
Grace Trace Ministries provides residential care and supported living for individuals with intellectual, developmental, and physical disabilities — including specialized DBMD care for deaf-blind individuals.
🌱Young Adults & Transitioning Youth
If you are aging out of foster care or transitioning to independent living — we provide life skills training, housing support, financial literacy, employment coaching, and a safe place to build your future.

Stabilization
Days 31 to 90

Development
Days 91 to 150
Build your housing plan. Increase savings. Search for permanent housing. Grow your independence.

Transition
Days 151 to 180
Secure permanent housing. Complete the program. Exit with a plan, savings, and support in place.

Move-In Day
Arrive with your documents. Complete intake. Sign your agreements. Get drug tested. Move into your own private room. Your new beginning starts today.
