Substance Use Recovery & Treatment Coordination
Grace Trace Ministries connects participants with qualified, licensed treatment professionals who provide individualized alcohol and substance use assessments, counseling, treatment planning, and ongoing recovery support.
Through our network of community treatment partners, participants may receive:
Substance use screenings and professional assessments
Outpatient or residential treatment referrals
Individual and group counseling
Relapse-prevention education
Peer recovery and accountability support
Medication-assisted treatment referrals, when appropriate
Coordination with probation, parole, courts, and reentry agencies
Continued case management and transitional housing support
Treatment services are provided by independent, appropriately licensed partner organizations. Grace Trace Ministries coordinates referrals, housing support, and continuity of care.
